Intensity Modulated Radiation Therapy (IMRT) shapes radiation beams to closely match the tumour — and varies the beam's intensity across the field, sparing more healthy tissue than conventional radiotherapy while allowing a higher dose to the tumour. VMAT delivers the same modulation in efficient arcs around you.

IMRT / VMAT · What it is

Intensity modulation, in plain language.

IMRT is a form of conformal radiation: the beams are shaped to closely approximate the tumour's outline. What sets it apart is modulation — the intensity of the radiation is varied across each beam during treatment, using multileaf collimators (MLCs) whose leaves move in and out of the beam's path as it's delivered.

A combination of these varying-intensity beams produces a highly sculpted three-dimensional dose distribution: maximum dose to the tumour, while reducing — or even avoiding — the healthy tissue beside it. Advanced computer planning finds the best combination of beams to achieve exactly that.

Because normal tissue is spared, a higher dose can safely be delivered to the tumour than conventional techniques allow. VMAT (Volumetric Modulated Arc Therapy) is the modern evolution: the machine delivers the modulated dose while rotating around you in continuous arcs, cutting treatment time substantially.

IMRT is used most extensively for cancers where dose needs to wrap tightly around critical structures.

Prostate & pelvic cancers

Tight dose shaping keeps bladder and rectal dose low across a full course of prostate, gynaecological or pelvic radiotherapy.

Head & neck cancers

Sculpting dose around salivary glands, swallowing muscles and the spinal cord — protecting voice, taste and swallowing.

GI, CNS, sarcoma & paediatric cancers

Gastrointestinal and central nervous system tumours, sarcomas, and selected paediatric cases where sparing growing tissue matters most.

Planning CT & contouring

A scan in the treatment position; your oncologist contours the tumour and every organ-at-risk slice by slice.

Inverse planning

The computer works backwards from the dose goals, calculating the MLC leaf patterns and beam intensities that achieve them.

Daily image-guided delivery

Each session: position, verify with imaging, treat. The MLC leaves dance through their pattern as the beam (or arc) delivers.

IMRT / VMAT · What to expect

Short daily sessions, normal life between.

Treatment is outpatient, typically daily on weekdays over several weeks depending on your prescription. Each session takes 10–15 minutes, most of it careful positioning; the beam itself is silent and painless.

Because side effects are reduced compared with conventional radiation, most patients continue working, driving and living normally through the course, resuming full activities earlier after it ends.

Site-specific effects — skin changes, fatigue, or irritation of nearby organs — are monitored at weekly reviews and managed as they arise.
